Heather Campbell has over 30 years experience of working in and leading Community Neurorehabilitation Services in South East London. The team are delighted to be returning to the Neuroconvention for a second year running.

As Clinical Lead & Head of Neurorehabilitation at Guys and St Thomas NHS Foundation Trust she leads and manages a large and expanding community based service which demonstrates innovation in service design- a collaborative team of Neuronavigators, networking across the 6 borough S.E London STP, a dedicated Stroke pathway inreaching to local acute stroke units and providing Early Supported Discharge, Community Stroke Rehabilitation and resettlement as well as 6 month stroke reviews, a Neurological Conditions Team supporting those living with Long Term Neurological Conditions including progressive illnesses, the Neurorehabilitation Enhanced Transition Team (NETT) providing intensive multidisciplinary rehabilitation as an alternative to hospital or specialist unit and a level 2b Neurorehabilitation Unit in the heart of Brixton.

Heather has contributed to several local and national guidelines on community stroke and neurorehabilitation services, led change in stroke across the capital under Healthcare for London and contributed to a National Audit Office model for stroke processes. The service has been recognised at local, regional and national level including twice being shortlisted for HSJ awards.

Community Neurorehabilitation in Lambeth and Southwark

Working across acute hospitals, specialist neurorehabilitation units and into the community, we have created collaborative partnerships with the patient at the centre. The service aims to support people to take control of their lives with an individualised approach to managing their recovery from stroke or brain injury or to live well with their neurological condition.
