The role of the care provider within the MDT
Premium Care Solutions are a Complex Care Provider, providing support to clients with various care needs across the UK. PCS work with a number of case managers, private clients and NHS CCG’s across the UK and we have over 16 years of experience in implementing packages of complex care. Our clients have varying support needs, including ventilated tracheostomy clients through to acquired brain injury clients. We are experienced in supporting clients’ transitions from hospital/care homes to returning home safely.
PCS shall be exploring the importance of the role of the Care Provider, within the MDT. We currently work extremely closely with all professionals involved in the rehabilitation of our clients. Our Seminar intends to explore this further, delving into topics such as evidence based care, management of care packages and how we can have a truly positive influence on the continuous development of our client’s rehabilitation at home.
